A powerful force of orcs, reinforced by mercenary wizards, attacked Wave Echo Cave to seize its wealth and the magical Forge of Spells within it. The dwarves, gnomes and human spellcasters of the Phandelver's Pact fought to defend the site, but the battle's own magic devastated much of the cavern, and the defenders — badly outnumbered — were overwhelmed. The cave's location was lost for five centuries afterward.
Cause
A deliberate military attack by orcs and allied mercenary wizards, seeking to seize the mine's wealth and the Forge of Spells by force, rather than an industrial accident.
What happened
Human wizards fought alongside their dwarf and gnome allies in defence of the Forge of Spells, but the resulting magical battle collapsed and destroyed much of the cavern around them. Overwhelmed, the Pact's defenders were wiped out, and the cave — its entrance buried and lost — passed out of history until rediscovered five hundred years later.
Aftermath
No authority is depicted as ever answering for the attack. The site's isolation and the manner of its defenders' deaths left it infested with undead and monsters, hazards still being dealt with by adventurers arriving five centuries afterward.
